The Health Equity Program: The Health Equity NP works as part of an integrated team with BQWCHC clients and community members with unmet health and social needs, including those unattached to primary health care, in a variety of settings (in-clinic, outreach clinic, and ‘home’-visiting activity). The NP provides barrier-free access to episodic and ongoing comprehensive primary care to individuals from priority, equity-deserving populations including newcomers, migrant workers, 2SLGBTQIA , seniors, and those living in poverty. Job Requirements
Qualifications:
* RN (EC) license to practice from College of Nurses of Ontario.
* Experienced as a Nurse Practitioner, with preference to those with experience in primary health care or in a community/outreach setting.
* Familiarity with addictions medicine and best practices as outlined by Meta:Phi Ontario, preferred.
* Experience working effectively with communities and individuals who face barriers accessing health care and other supports, including those with problematic substance use, mental health, and complex psycho-social issues.
* Ability and willingness to work in an unstructured clinic/street-based model, while maintaining vigilance for the importance of the strategic goals of the program.
* A valid driver’s license; reliable vehicle and willingness to travel within the Quinte area is required.
