Job Description
The Nurse Practitioner works within an inter professional team to provide primary health care to clients. The NP participates in the development, implementation, monitoring and evaluation of primary care services for individuals, families and the community. KEY RESPONSIBILITIES:
• Maintain and manage a caseload of varying ages
• Primary Care Services: assessment, diagnoses, treatment, chronic disease prevention and management, health promotion and counseling
• Collaboration and consultation with physician as required
• Maintain accurate and current client documentation using electronic medical records system.
• Participate in case conferences, team and staff meetings
• Work within an inter-professional team to provide quality evidenced based client care
• Working in a manner that incorporates a health promotion approach supporting the social determinants of health
Job Requirements
1. Baccalaureate or Master of Nursing degree with successful completion of a post graduate Primary Care Nurse Practitioner program
2. Good standing with the College of Nurses
3. Current membership with NPAO, including professional liability insurance
4. Experience with Primary Care, preferably in a community setting
5. Experience with pediatrics and infant care considered an asset
6. Demonstrated commitment to an inter-professional team environment
7. Comfort with electronic medical record systems
8. A valid driver’s license, and auto insurance
