Job Description

Gateway Community Health Centre is seeking two (2) Nurse Practitioners for an exciting initiative in the Central Hastings and Belleville communities serving clients who are currently awaiting a Primary Health Care Provider. This six-month primary care-led pilot project will create functionality and pathways for unattached patients to access care and services. This project will be implemented at two primary health care sites, Gateway Community Health Centre in Tweed, and Belleville Nurse Practitioner-Led Clinic.

Job Requirements

Responsibilities: • Identification of unattached patients • Development of appropriate patient-centred care plan considering the need for episodic or preventative care and chronic disease management • Perform holistic health assessment and history, physical examination based on client needs and across the lifespan, through in-person and virtual visits • Use strong clinical reasoning skills, and evidence-informed practice to formulate differential diagnoses and communicate findings to patient • Collaborate with patients and other providers to establish priorities and ensure coordination of the care plan. • Identify issues and trends related to patient group health issues and use the information in planning and advocacy work • Maintain clear, accurate, and timely documentation including requisitions, reports, and encounters as required in an EMR • Reporting and evaluation of project deliverables to the Hastings Prince Edward OHT Qualifications: • Current registration with the College of Nurses of Ontario as a Nurse Practitioner with PHCNP specialty certificate, current NPAO membership and evidence of liability insurance required • 5 years’ experience working in Primary Care settings, community health setting preferred • Completion of “Prescribing Narcotics and Controlled Substances” (NPAO courses preferred) • Experience and/or openness to working with individuals who face barriers accessing traditional health care institutions • Demonstrated experience as a nurse leader with excellent organizational, critical thinking, communication, and interpersonal skills • Respectful of the values and diversity of the communities and individuals we serve • Demonstrated ability to be flexible while working in a fast-paced, ever-changing environment This position requires flexibility in hours of work to support accessible program service delivery at the hosting sites and as such, may require evening and or weekend work. The salary range for this position is $87,953.00 – 122,178.00. Gateway CHC is an inclusive work environment.
