Your Opportunity!
Knollcrest Lodge is seeking a Part-Time (3 days a week) or Full-Time Nurse Practitioner to join our team in Milverton. In this role, the NP will work closely with residents, their families or caregivers, and the healthcare team to develop, implement, and evaluate personalized care plans. Additionally, the NP will mentor staff to enhance their knowledge and assessment skills. This position reports directly to the Director of Resident Care and the CEO, and is accountable to the Medical Director.
Key Responsibilities:
– Provide primary care to residents within the scope of practice set by the College of Nurses of Ontario (CNO)
– Engage in research, education, and evidence-based initiatives to improve resident care and health system outcomes
– Conduct comprehensive health assessments, diagnose acute and chronic conditions, prescribe treatments, and order diagnostic tests
– Provide palliative and end-of-life care and ensure a smooth transition for residents moving between care settings
– Collaborate with physicians, nursing staff, and external healthcare providers to optimize resident care
– Document clinical findings, interventions, and care plans accurately and in a timely manner
– Engage in research, applies evidence-based practices, and acts as a change agent to enhance care quality and outcomes
– Other duties, as assigned
Job Requirements
– Must be a Registered Nurse (RN) in the Extended Class (Primary Health Care or Adult) with current CNO registration
– Provide after-hours and on-call coverage in accordance with the Doctor’s needs
– Recommended qualifications include postgraduate education in gerontology, specialty certification in gerontological nursing, and palliative care experience
– Must maintain confidentiality, ethical conduct, professional liability protection, CPR certification, and ongoing clinical competence
– Ability to read and write in English, and understand verbal and written instructions in English
– Strong team player with the ability to cooperate with colleagues and department heads
– Excellent communication skills with the ability to effectively collaborate with other colleagues
