Overview:
The Loyalist Family Health Team (LFHT) in Amherstview, Ontario, just minutes from the shores of Lake Ontario, is seeking a skilled, enthusiastic, dedicated Nurse Practitioner interested in collaborative team-based primary care. The Nurse Practitioner will help to increase access to primary care for patients by focusing on acute and episodic care as well as patient care across the life span. This includes, but is not limited to, acute and chronic disease management, preventative care, prenatal visits, well-baby care, health teaching, prescription renewals, and other practitioner-patient interventions. This exciting opportunity will appeal to a self-directed professional interested in practicing full scope and providing excellent team-based care to a wide variety of patients in collaboration with a group of supportive physicians and committed interdisciplinary health care providers.
Main Responsibilities:
▪ Diagnoses and treats clients for acute episodic illnesses
▪ Orders appropriate investigations; interprets reports of investigations and analyzes information to monitor progress and plan treatment
▪ Applies knowledge of pharmacology, antibiotic stewardship, and evidence-based practice in appropriately selecting, prescribing, administering, and monitoring the effects of drugs
▪ In partnership with other providers; support and counsel patients, establish priorities for management of health conditions, provides follow-up treatment, communicates with patients and families about health findings

Job Requirements
Education, Experience and Skills:
▪ Current extended class certificate and unrestricted registration with the College of Nurses of Ontario (Masters or NP-PHC are considered an asset)
▪ Current membership with RNAO, including professional liability insurance program
▪ At least one (1) year of clinical experience is preferred, particularly in primary care
▪ Ability to work effectively both independently and as part of a team
Job Requirements:
▪ This position requires the successful candidate to work onsite, and we provide a dedicated office and examination room for these purposes
▪ Willingness to work one evening per week is strongly preferred
▪ This position is available immediately
