| JOIN OUR TEAM Nurse Practitioner –Permanent Full-Time Position This new position is part of the recently announced Homeless and Addiction Recovery Treatment (HART) Hub Niagara. This position will be working with individuals faced with homelessness, substance misuse, and mental health concerns.Quest Community Health Centre Quest CHC is a dynamic non-profit, interdisciplinary health care organization. Job Requirements
The ideal candidate will maximize connections and use of existing community resources by acting as a catalyst who, together with community stakeholders, will facilitate the development and implementation of creative and efficient strategies to maximize health outcomes for our clients, while maximizing the use of community care plan.
