Job Description

A NP/CNS vacancy exists in Diabetes and Endocrine Clinic: St. Joseph’s Hospital continues to expand its role the management of complex medical and chronic disease, illness prevention, research and education. Interdisciplinary teams provide comprehensive assessment, diagnosis, and treatment and follow up care through our specialty programs. The successful candidate will join the interdisciplinary team in supporting our patients in their quest to live fully, and will liaise with internal and external community partners to provide effective and compassionate care. An exciting opportunity exists at St. Joseph’s Hospital for a Nurse Practitioner/Clinical Nurse Specialist (NP/CNS) to provide services for the patients of medicine services, specifically to patients with diabetes in the Diabetes Education Centre and the Diabetes and Endocrinology Clinic. The successful applicant will also support program development and provide care to patients with other chronic conditions as integrated clinical care pathways are expanded. The NP/CNS will work within a collaborative practice model and comprehensive inter-professional team. The NP/CNS will provide direct, specialized, comprehensive care and provide clinical consultation to the interdisciplinary care team regarding patient, family and professional practice issues. The NP/CNS will provide leadership at the program, corporate and professional level. The NP/CNS will also provide clinical practice leadership and set clinical direction to the inter-professional team by incorporating best practice and research findings to promote quality patient care. The NP/CNS will also lead and participate in the education of the care team through on-going; evidence based clinical training, mentoring, coaching and practice audits. A NP/CNS vacancy exists in Chronic Obstructive Pulmonary Disease (COPD) Clinic/Pulmonary Rehabilitation: St. Joseph’s Hospital continues to expand its role in the treatment of complex medical and chronic disease, secondary illness prevention, research and education. Our ambulatory medicine teams specialize in the treatment of complex medical and chronic disease conditions with the goal of assisting individuals to reach optimal health and well-being. Our different teams work together to provide patients with comprehensive assessment, diagnosis, disease prevention and management strategies and follow up care. St. Joseph’s offers a comprehensive interprofessional COPD/Pulmonary Rehabilitation program including medical support, exercise training, dietetic counseling, patient education, social work support, respiratory therapy and self-management techniques. Patients work with members of the team to create and follow an individualized plan of care with the focus of enabling patients to identify and control risk factors that contribute to COPD. This program is currently seeking a Nurse Practitioner/Clinical Nurse Specialist (NP/CNS) to provide comprehensive patient care by using advanced assessment, intervention and evaluation skills related to COPD and other respirology disease such as asthma. The NP/CNS will be responsible and accountable for developing and coordinating an individualized care plan in collaboration with the physician (respirologist, general internist, etc), the care team, patient and family. The NP/CNS will also provide clinical practice leadership and set clinical direction by incorporating best practices and research findings that promote quality patient care.
