St. Joseph’s Hospital continues to expand its role the management of complex medical and chronic disease, illness prevention, research and education. Interdisciplinary teams provide comprehensive assessment, diagnosis, and treatment and follow up care through our specialty programs. The successful candidate will join the interdisciplinary team in supporting our patients in their quest to live fully, and will liaise with internal and external community partners to provide effective and compassionate care. An exciting opportunity exists at St. Joseph’s Hospital for a Nurse Practitioner/Clinical Nurse Specialist (NP/CNS) to provide services for the patients of medicine services, specifically to patients with diabetes in the Diabetes Education Centre and the Diabetes and Endocrinology Clinic. The successful applicant will also support program development and provide care to patients with other chronic conditions as integrated clinical care pathways are expanded. The NP/CNS will work within a collaborative practice model and comprehensive inter-professional team. The NP/CNS will provide direct, specialized, comprehensive care and provide clinical consultation to the interdisciplinary care team regarding patient, family and professional practice issues. The NP/CNS will provide leadership at the program, corporate and professional level. The NP/CNS will also provide clinical practice leadership and set clinical direction to the inter-professional team by incorporating best practice and research findings to promote quality patient care. The NP/CNS will also lead and participate in the education of the care team through on-going; evidence based clinical training, mentoring, coaching and practice audits.
Requirements
Essential Qualifications- Valid registration with College of Nurses of Ontario as a Registered Nurse in the Extended Class
- Masters of Science in Nursing (MScN) or Masters of Nursing (MN) or health care related Masters degree
- Primary Health Care or Adult Nurse Practitioner Specialty certificate
- Certified Diabetes Educator
- Member of RNAO or NPAO with professional liability insurance
- Minimum three years of recent clinical experience in patient diabetes management
- Demonstrated knowledge, skills and expertise in best practice patterns and translating them into clinical practice as it relates to Diabetes Canada’s Clinical Practice Guidelines for the Management of Diabetes
- Demonstrated experience in project management, specifically related to education and quality improvement
- Advanced knowledge and skills with a multi-system focus in physical assessment, consultative support and evaluation specific to the care of patients with diabetes
- Superior interpersonal, coaching and communication skills to effectively work within an interprofessional team setting
- Demonstrated critical thinking and problem solving skills
- Demonstrated creative initiatives in care planning, problem-solving, conflict management, and independent handling of priorities and emergencies
- Comprehensive knowledge of and ability to mobilize community resources
- Ability to demonstrate a facilitative teaching approach with patients/families and staff
- Demonstrated experience in conducting and/or promoting research use in practice
- Previous teaching experience
- Demonstrated ability to provide clinical leadership
Immunization Requirements- Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
- Provide documentation of the Tuberculosis skin testing
