St. Joseph’s Hospital continues to expand its role in the treatment of complex medical and chronic disease, illness prevention, research and education. Our ambulatory medicine teams specialize in the treatment of complex medical and chronic disease conditions with the goal of assisting individuals to reach optimal health and well-being. Our different teams work together to provide patients with comprehensive assessment, diagnosis, disease prevention and management strategies and follow up care. The Heart Failure Clinic provides care within an inter-professional model for patients living with heart failure. Patients work with members of the team to create and follow an individualized plan of care with the focus of enabling patients to identify and control risk factors that contribute to heart failure. This program is currently seeking a Nurse Practitioner/Clinical Nurse Specialist (NP/CNS) to provide comprehensive patient care by using advanced assessment, intervention and evaluation skills related to heart failure and cardiac rehabilitation. The NP/CNS will be responsible and accountable for developing and coordinating an individualized care plan in collaboration with the Cardiologist, the care team, patient and family. The NP/CNS will also provide clinical practice leadership and set clinical direction by incorporating best practices and research findings that promote quality patient care
Job Requirements
Essential Qualifications Valid registration with College of Nurses of Ontario as a Registered Nurse Extended Class with no restrictions
Masters of Science in Nursing (MScN) or Masters of Nursing (MN)
Nurse Practitioner certificate in either Primary Health Care or Adult
Member of RNAO, NPAO, Canadian Nurses Protective Society (CNPS) with professional liability insurance
Minimum three years of recent clinical experience in ambulatory care
Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
Ability and commitment to act as a supervisor under OHSA
Demonstrated knowledge, skills and expertise in the area of cardiac rehabilitation and the clinical management of heart failure
Demonstrated knowledge of chronic disease management and the promotion of self-management
Superior interpersonal, coaching and communication skills to effectively work within clinic settings
Demonstrated critical thinking and problem-solving skills
Ability to demonstrate a facilitative teaching approach with patients/families and teams
Demonstrated experience in conducting and/or promoting research use in practice
Demonstrated leadership experience
Preferred Qualifications
Proficiency in French would be an asset

Immunization Requirements
Provide documentation you have received two doses of the Covid-19 vaccine and a written commitment to follow any future Public Health recommendations and St. Joseph’s Health Care London policies and processes related to Covid management in the workplace.
Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
Provide documentation of the Tuberculosis skin testing
