Job Description

Position Summary: The Nurse Practitioner (NP) is a third party contractor and forms part of the interdisciplinary management team with the role of practitioner, consultant, educator and advocate. The NP practices autonomously, offering the full scope of primary health care nurse practitioner practice. He/she functions as a member of the interdisciplinary care team in the development, implementation and evaluation of the resident’s plan of care. The Nurse Practitioner will collaborate with the Medical Director, other physicians and community services and agencies in the provision of primary health care for residents. The Nurse Practitioner will collaborate with the Directors of Care in the provision and advancement of nursing services, taking a leadership role in the development, implementation and evaluation of the Nursing department. Accountabilities: – Provides comprehensive primary care to residents as part of the interdisciplinary health care team. – Manages the care of residents by providing pharmacological, complementary, non-pharmacologic and/or other interventions within the scope of practice for NPs – Conducts routine in person rounds in the LTC home to provide acute clinical care, preventative assessments, and improve quality of life for those with chronic conditions. – Documents clinical data, assessment findings, diagnoses, plans of care, therapeutic interventions, resident responses and clinical rationale in a timely and accurate manner. – Identifies circumstances in which palliative and/or end of life care may benefit the resident and family members and work collaboratively with members of the interdisciplinary team. – Works effectively and efficiently with members of the interdisciplinary team including the Medical Director to ensure the highest level of care is provided to residents. – Provides leadership in developing and implementing strategies to optimize the integration of illness and injury prevention, health promotion, health maintenance, rehabilitation and restorative care activities. – Participates in creating an organizational environment that supports safety and quality of resident care and life, collaborative practice, and professional growth. – Identifies, develops and implements practice innovations, in collaboration with the residence’s senior leadership team.

Job Requirements

– Baccalaureate in Nursing from a recognized university. – Current and valid registration in the Extended Class with the College of Nurses of Ontario.
