The Toronto Hypertension Clinic, established in 2023, has quickly become a regional center of excellence in hypertension management. Led by Dr. Liam Finlay, an internal medicine and certified hypertension specialist, we proudly serve a diverse population referred by family physicians, emergency departments, and specialists. Our clinic offers a collaborative, supportive environment, with opportunities to work closely alongside internal medicine, nephrology, and cardiology experts.

Position Overview
We are seeking a skilled and compassionate Nurse Practitioner (NP) to join our growing team. In this role, the NP will provide follow-up care for patients with hypertension, working closely with the physician to develop and implement individualized treatment plans. This is an exciting opportunity to practice in a dynamic clinical setting and make a significant impact on patient care.

Key Responsibilities

  • Conduct patient assessments, interval histories, and physical examinations as needed.
  • Review laboratory results, cardiac imaging, and blood pressure monitoring logs.
  • Create and document follow-up notes using structured templates in the EMR.
  • Educate and counsel patients on hypertension management, cardiovascular risk reduction, lifestyle modifications, diet, and exercise.
  • Collaborate with the supervising physician, reviewing complex cases as needed.
  • Prepare bloodwork and diagnostic requisitions, and write prescriptions within scope.
  • Effectively communicate treatment plans to patients and their families.

Qualifications & Requirements

  • Current registration and in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse in the Extended Class (NP).
  • Primary Health Care Nurse Practitioner Certificate or equivalent recognized NP program.
  • Master’s degree in Nursing or equivalent education and training.
  • Minimum of 1 year of experience as a Nurse Practitioner, preferably in internal medicine, family medicine, cardiology, or chronic disease management settings.
  • Demonstrated experience in patient counseling and education.
  • Strong clinical decision-making and problem-solving skills.
  • Excellent verbal and written communication skills.
  • Familiarity with Accuro EMR or similar systems is an asset.
  • Ability to work collaboratively in a team setting and independently manage a full patient caseload.

Competitive hourly wage: $70 per hour
4 weeks of paid vacation per year
CME Support: 3 paid days for continuing medical education and a $1,000 CME budget
Personal office space and access to Dragon voice dictation software
Free on-site parking and easy transit access
Supportive and collegial team atmosphere

Work Schedule
Monday to Friday
8:00 AM – 4:00 PM (half-hour unpaid lunch)
37.5 hours per week
