Job Description
Position: Nurse Practitioner – Primary Health Care Reports to: Coordinator of Addiction Medicine Programs Position Summary The Nurse Practitioner (NP):
- Is an integral member of Addiction Services of Thames Valley (ADSTV), working with addiction counsellors, nurses, primary care providers and other community health service providers in offering addiction medical support of individual clients living with mental illness and addiction through a harm-reduction lens
- Functions interdependently with staff members to provide shared, client-centered care that emphasizes health promotion/protection, disease prevention and client education.
- Provides episodic primary health care to ADSTV clients who do not have a primary care provider and consults with collaborative physicians for clients who have unstable/complex health conditions

- 35 hours/week – may include some evenings or Saturdays as required

- Schedule may vary according to program needs
Compensation
- $107,635/annum based on a 35 hour work week
- Benefit eligibility after 3 months
- Participation in Healthcare of Ontario Pension Plan (HOOPP)
Duties and Responsibilities:
- Maintain requirements for practice, including a license for a Registered Nurse in the Extended Class, NP/Physician Collaborative Practice Agreement, and malpractice insurance.
- Maintain confidentiality and act in an ethical manner, consistent with professional expectations and the College of Nurses of Ontario (CNO).
- Retain clinical competence through relevant evidence-based research and legislation.
- Proficient in computer skills and experience with electronic medical record systems (i.e. PS Suites).
- Demonstrate ability, commitment to and knowledge of community addictions and mental health care.
The Nurse Practitioner will function in a variety of settings: A. Rapid Access to Addiction Medicine (RAAM) Clinics:
- Exhibit knowledge of evidence-informed guidelines, legislation, regulation and organizational policies when prescribing controlled drugs/substances (CDS).
- Demonstrate an understanding of best practices related to the use of CDS and of other pharmacological and non-pharmacological therapeutic options.
- Excellent communication, decision-making, problem-solving, conflict management and time management skills.
- Demonstrate ability to work effectively in an interdisciplinary team environment.
B. Health Care Services to ADSTV / RAAM clients with no health care provider:
- Provide health care services to clients 12 years and older that meet eligibility.
- Provide leadership and guidance for the development of expanded medical services at ADSTV
- Perform comprehensive care using advanced health assessment, physical examination, therapeutic management, consultation, collaboration and referrals in accordance to the Practice Standard: Nurse Practitioner (2018).
- Work in collaboration with consulting physician and the inter-professional team to provide client-centred health care services to meet the needs of ADSTV clients.
- Maintain client records for accuracy and confidentiality.
- Support the health needs of individuals with a history of addictions, mental health and other chronic illnesses and refer to specialists and other community agencies.
- Demonstrate ability to deal effectively with sensitive issues with clients and families using a harm-reduction and client-centred approach.
- Determine the need for follow-up appointments and assist with transfer of care to permanent primary health care services.
C. CWSP / Outreach & Community Partnerships:
- Work in partnership with Community Withdrawal Support Program (CWSP), community agencies and programs.
- Integrate ADSTV counselling services and support for clients.
- Coordinate service provision with other health care partners in the development of community care plans and wrap-around services.
- Provide inter-discipline home visits with vulnerable clients or with clients with transportation and other barriers to care services when required
- Assist in program development specific to CWSP that align with best practice standards, evidence-based research and ADSTV program mandate.
- Promote the integration of addiction-based medicine and primary care
- Offer educational opportunities and consultation as appropriate
Job Requirements
- Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class) (Proof of registration required)
- Master’s Degree (Strongly recommended)
- Minimum of two to three years of community health care experience (Required)
- Experience in addiction medicine and harm reduction (Strongly recommended)
- Completes 6-hour Suboxone on-line education program (Recommended)
- Current certificate in cardiopulmonary resuscitation (CPR) (Required)
- Successful completion of the Prescribing Narcotics and Controlled Substances course (Copy of certification required)
- Current Vulnerable Sector Check within past 3 months (Required)
- Current Ontario driver’s license and reliable transportation
- Excellent clinical, interpersonal and organizational skills
- Project planning and evaluation skills would be an asset
