The good news is that just north east of Toronto, in the heart of cottage country, Haliburton County is ready to welcome a nurse practitioner into our FHO.  We are a group of 8 collegial physicians and two Nurse Practitioners serving families in the beautiful Haliburton Highlands at the Haliburton Family Medical Centre. We are also affiliated, and work collaboratively with the Haliburton Highlands Family Health Team. We currently have a roster of 10,080 patients.  The Nurse Practitioner (RN(EC)) provides acute/episodic/ongoing chronic care, and supports the primary health care needs of the community within the scope of practice and standards of care of a Nurse Practitioner as outlined in the College of Nurses of Ontario’s – Standards of Practice for Registered Nurses in the Extended Class. The NP diagnoses and manages common health problems and diseases both independently and in collaboration with the client and the physicians. The NP will have specialized skill in gerontological care specifically to treat physical, emotional, cognitive, and social needs of geriatric patients. The NP works as part of an interdisciplinary team to address issues in the community that affect health. Accountability: The NP is to practice within the scope and according to the standards of practice as outlined in the following documents: College of Nurses of Ontario’s ~ “Practice Standards – Nurse Practitioners” (www.cno.org/docs/prac/41038_StrdRnec.pdf) ď‚· Expanded Nursing Services for Patients Act (1997) ď‚· Nursing Act (1991) ď‚· Regulated Health Professions Act (1991) ď‚· Health Protection and Promotion Act (1990) Duties and Responsibilities: Patient Care ď‚· Provide acute/episodic and ongoing chronic care, primarily to unattached, poorly attached, and equity deserving patients in the community ď‚· Provide coverage for physician absences, including same day appointments and reviewing labs, consult notes, diagnostics test results, etc. ď‚· Screening for the presence of chronic disease ď‚· Monitor the ongoing therapy of patients with chronic stable conditions ď‚· Formulate and communicate medical diagnoses and disorders Referral/Consultation/Collaboration ď‚· Consult with physicians in accordance with the standards for physician consultation ď‚· Liaison with other team members as required ď‚· Arrange for external specialist consultations ď‚· Provide communication to hospital and community staff and other inter-professional team members as required to ensure smooth transition for care Administration ď‚· Arrange follow-up appointments as necessary ď‚· Code visits daily as per Schedule A – Programs and Services ď‚· Other Activities –Other activities may be assigned in collaboration with the inter-professional team SALARY: Salary will commensurate with experience. Job Requirements

SPECIFICATIONS EDUCATION / EXPERIENCE:  Baccalaureate or Masters in Nursing  Successful completion of post-graduate Primary Care Nurse Practitioner program  Current extended class registration with the College of Nurses of Ontario  Current membership with RNAO, including professional liability insurance program  Basic CPR certification  Current Ontario Drivers’ License  Demonstrated experience in Community and/or Primary Care Setting  Demonstrated experience in the area of chronic disease management and prevention SKILLS / ABILITIES:  Knowledge and understanding of:   Canadian and Ontario Health care system Role of the NP within an interdisciplinary team, how to optimize collaborative relationships The patient as an individual, a family member and a member of the community Issues specific to the residents of Haliburton Highlands and resources available  Knowledge and proficiency in current, evidence-based methods and practices of primary care, with an emphasis on heath promotion and risk reduction.  Superior leadership, organization, research, evaluation, time management, communication and interpersonal skills.  Proficiency in the use of the computer hardware and software, particularly in Microsoft Word, Excel, Outlook.  Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences.  Multi-tasking skills related to the delivery of efficient primary care including the ability to share information and teach while treating a patient.  Excellent verbal and written communication skills.  Exhibit an ability to be open and non-judgmental.  Knowledge of the goals and structure of the organization, the policies and procedures and the programs.
