The Nurse Practitioner (NP) provides direct patient care within residential Withdrawal Management and Treatment Services (WMTS) to the full scope of practice for an NP. You will work within a multidisciplinary team to provide comprehensive biopsychosocial support for individuals with addictions and concurrent disorders. This includes clients in acute withdrawal from one or more substances. Your focus will be on comprehensive health assessment, individualized treatment plans, and supporting continuity of care through collaboration and referrals. The NP at WMTS provides episodic primary care to clients in program and assists with transfer of care to more permanent services. This position will collaborate with Agency leadership to inform program evolution to ensure highest quality of care possible for our vulnerable clients. The NP will also provide guidance and coaching to other members of the WMTS team, including non-medical staff, to ensure best service possible. You make a difference! The WMTS Nurse Practitioner:
– Adheres to CPSO prescribing guidelines, CNO Standards of Practice and demonstrates practice competencies
– Conducts comprehensive health history, mental health/addictions/concurrent disorders assessment and physical assessment with identification of health needs and formulation of evidence-informed plan of care
– Supports the WMTS interdisciplinary team to implement plans of care, including transition plans, to best serve clients
– Enacts clinical leadership skills to organize and coordinate the care of clients in collaboration with the WMTS team and external partners (i.e. pharmacy, RAAM medical team, hospital) to ensure safe and effective practice and continuity of care with internal and external partners
– Prescribes and administers medication to support safe and comfortable withdrawal and monitors the impact of prescribed medications on clients in withdrawal and their impacts on clients’ mood, behavior, cognitive processes
– Conducts risk assessments for suicide, self/harm or harm to others and takes the necessary action
– Provides leadership in advancing evidence-based addiction and concurrent disorder services
– Works from a harm reduction, patient-centred, strengths-based approach
– Supports education, collaboration and knowledge exchange to enhance partner, primary care and community capacity to effectively address addiction
– Collaborates with interdisciplinary support team and external partners (i.e. pharmacy, RAAM medical team, hospital) to ensure safe and effective practice and continuity of care Annual salary: $118,404 plus shift premiums, benefits and initial signing bonus
Hours of work: 37.5 hours per week – schedule to be negotiated with successful candidate.
Job Requirements
TO APPLY, YOU MUST:
– Be registered in Good Standing with the Ontario College of Nurses – Extended Class License
– Possess a Bachelor’s degree in Nursing, Master’s degree preferred
– Have successfully completed a Nurse Practitioner program from a recognized university
– Have a minimum of three years’ related clinical experience in mental health and/or addictions setting, with knowledge of addiction medicine and harm reduction
– Demonstrate knowledge of CNO Standards of Practice and commitment to maintaining reflective practice standards as described by CNO
– Demonstrate ability to conduct advanced focus or comprehensive health history, mental health/addictions/concurrent disorders assessment and physical assessment with identification of health needs and formulation of evidence-informed plan of care
– Have completed CNO approved education for prescription of controlled substances
– Be knowledgeable in trauma-informed practices and motivational interviewing techniques for the concurrent disorders population
– Demonstrate knowledge of Canadian Standards of Psychiatric-Mental Health Nursing, as well as relevant legislation, including the Mental Health Act, Health Care Consent Act, and Personal Health Information Protection Act (PHIPA)
– Possess clinical leadership skills to organize and coordinate the care of clients
– Have current certificate in cardiopulmonary resuscitation (CPR)
– Have access to reliable transportation
– Be able to manage frequent exposure to direct and vicarious trauma
