She has established and leads a low-barrier clinic within the Homeless Day Hub, intentionally designed as a space grounded in dignity, respect, and accessibility. Her approach breaks down stigma and fear, allowing people who have often been excluded from the healthcare system to receive care in a way that feels safe and affirming.

In her role, Heather provides real-time clinical direction and support to a multidisciplinary team, ensuring that each member practices to their full scope and feels confident responding to complex and often urgent care needs. Her leadership is calm, steady, and grounded in shared purpose: promoting collaboration, professional growth, and a deeply compassionate care environment.

The outcomes from Heather’s work are profound. The clinic is already seeing limb preservation through timely wound care, critical naloxone interventions that have saved lives, and a significant diversion of EMS calls and emergency department visits, resulting in improved patient outcomes and substantial system-level impact. These efforts not only change the trajectory of individual lives, but also allow healthcare resources to be redirected to help even more people.

Heather’s work challenges inequity and advances social justice in healthcare.
