As the people of Ontario look forward to a future that includes access to Primary Care for All, Nurse Practitioner-Led Clinics (NPLCs) represent an exceptional opportunity. NPLCs are a uniquely designed, Ontario-based model of ministry-funded, team-based primary care that improves client health outcomes and delivers positive care experiences.

NPLCs stand apart from other primary care models in that Nurse Practitioners (NPs) are the Most Responsible Providers (MRPs), which means they are the healthcare provider who has overall responsibility for directing and coordinating care for their patients. NPs assess clients’ health concerns, diagnose conditions, plan and implement treatments, perform necessary procedures, and coordinate care within the interdisciplinary team and with community and medical support. NPLCs maximize the scope of practice for all team members, ensuring high-quality care, positive client experiences, and timely access to services across the province.

NPLCs play a vital role in keeping Ontarians healthy by providing reliable access to care, reducing the burden on our healthcare system, and diverting unnecessary visits to emergency rooms and hospitals, reserving emergency services for true emergencies. Integrated into their communities, NPLCs focus on population health management and wrap-around care. Remarkably, they provide this comprehensive care at a fixed cost of approximately $500 per enrolled person per year when operating at capacity.

This cost reflects the full continuum of care provided to enrolled patients, including operations, staff compensation, and in-person bricks-and-mortar expenses. Additionally, this figure does not account for the broader impact of NPLCs, as many clinics also deliver programs and services to non-enrolled (at no-cost) patients within their communities.

It is important to highlight that the fixed cost per patient in an NPLC encompasses far more than just primary care. It always includes access to a primary care provider, as well as many of the following; mental health and addictions support, chronic disease management, dietitian supports; and services such as nursing care and foot care, where applicable. These services are often provided without tying up appointments with the MRP, optimizing care delivery and resource utilization. This comprehensive and integrated model distinguishes NPLCs from other primary care models and underscores their value.

NPLCs are one of Ontario’s best-kept secrets, and every clinic is ready to expand and support the goal of ensuring every Ontarian has access to a primary care home. NPLCs also leverage the NP clinician and leadership role to advance their proven model of care, NPLCs are well-positioned to be a cornerstone of Ontario’s healthcare future, delivering accessible, high-quality, and cost-effective primary care to the people who need it most.

NPLCs have been demonstrating their cost-effective and exceptional value for care in Ontario since 2007. It all started with one clinic site in Sudbury and has now grown to 27 independently funded clinics, many with multiple sites, that span the entire Province. Not only have NPLCs demonstrated excellence in managing patient care throughout the lifespan but they have also leveraged leadership roles to engage with stakeholders at the local, provincial and national level to impact healthcare delivery for Ontarians and Canadians.
