  1. Approval of the minutes – December 12, 2018 – posted on the Portal
  2. Happy New Year to All – review joint progress and priorities:
    1. Review strategic plan
    2. Opportunity for feedback, suggestions re: change to priorities
    3. Priority for end of fiscal – “Marketing and branding” strategy; advocating for ministry funding enhancements and support
    4. Glengarry – are any clinics looking at external funding mechanisms – i.e. charitable organizational status? Some clinics have been applying for grants to offset costs r/t capital; Twin Bridges has asked auditor re: charity and suggestion was to not explore; if any clinics explore further funding options or charitable status, please share learning
  3. Updates from NPAO – Claudia
    1. Provincial Advocacy work – power point presentation provided
    2. Reviewed advocacy work current and ongoing, requesting feedback regarding priority work and next steps
      1. NPAO is also requesting information from clinics regularly that can be posted on line – highlights, quality care, quotes, programs, etc
    3. New research opportunity – “Impact of Primary Care Teams’ Funding Model on Roles, Processes and Comprehensiveness” – NPAO/NPLCA applying for joint funding with Canadian Institute for Health Research (CIHR) – reach out if you would like to be involved
  4. PCAM Study update – Stephanie
    1. Has been working with project group to determine next steps
    2. Have reached out to researcher Roberta Heale who has expressed interest in involvement, exploring funding mechanisms
    3. Working on providing a summary document and what the commitment would be at the clinical level – consent, overview, application of PCAM tool – wondering about the role of the QIMSS to complement
    4. Stephanie will provide an overview of the project for distribution and sharing with the NPLCs
  5. 2019 NPLC Leadership Conference Planning Update
    1. June 3rd and 4th – Holiday Inn Toronto Airport
    2. THEMES – NPLC sustainability – Proposed guest – Michele Acorn PCNO
      1. Growth of NPLCs
      2. Funding freezes – how to sustain the clinics in fiscal restraint
    3. Hallway medicine – developing a united message
    4. Marketing 101 – guest speaker marketing
    5. Registering & Rostering – Nadia Surani – plan for movement
    6. Collaboration and Quality- Thinking outside the box:
      1. NPLC presentations (10 min sessions each) – suggested –
        1. Sudbury NPLC – community stakeholder meetings
        2. Ingersoll NPLC – developing partnership agreements
        3. Paula/Sue/Kathy H – proposal on RACC clinic
        4. Open sessions x 3
      2. Quality Session – QIIMS to discuss quality across all NPLCs

      4. Sponsorship – comments – may be an option but that would need to be set up separately from main hall; do not want presentations to interfere with conference work
    7. NP Lead Role Review – Jenn
      1. Survey from Michelle Clifford-Midel
        1. Jenn has now compiled documents to examine NP Lead role in the clinics – meeting tonight to review
        2. Jenn has been keeping compiled results across the province to set some guidance documents for provincial work
  • Michele – similar requests as her board is attempting to gather cross clinic basis as to the workload, patient flow patterns

  1. QIMSS and Data Sharing Agreements – progress update
    1. Host Georgina – all NPLCs have signed – first meeting next week
    2. Host Sudbury – two clinics have yet to sign agreements – meeting next week
    3. Host Ingersoll – one clinic has yet to sign
    4. Three QIMSS continue to meet on a weekly basis and have developed a set of common indicators that will be collected across clinics; are also meeting with HQO regarding new indicators; updates to be provided regionally; invite to the next meeting to prepare sites for 2019/20 QIP year
  2. HQO/QIP communication – what’s coming 2019/20
    1. Communication received December 12 – available on HQO website
    2. “Fewer and more focused priorities and ambitious targets”
    3. “Patients newly dispensed an opioid within a 6-month reporting period/total “roster” of the clinic” – this will generally be traceable externally, however NPLCs will need to develop queries.
      1. QIMSS meeting with HQO to discuss this and how NP data should be collected and reported
    4. “Proportion of patients who were identified at risk of dying and in need of palliative care in the last 6 months and had documented assessments of their palliative care needs in their EMR” –will need standardized process for collection and reporting
    5. Question Glengarry – is there appetite to go “above and beyond” with QIP indicators?
      1. Feel is that HQO is interested in seeing improvement – i.e. if something is working well, don’t invest a lot of time on that issue – focus on where things need to improve
      2. Some of this information will come forward with QIMSS reports – more information TBD
    6. CAHBI Project Feedback – next steps
      1. Many clinics have trialed the dementia assessment tool
      2. Final report is due by NPAO at the end of March
      3. NPAO will be distributing a short assessment questionnaire before the end of January
      4. If you have not had a chance to try this tool, please do as it helps both clients and the association to secure funding in the future through similar projects
    7. Provincial updates/roundtable
      1. Glengarry – OHA – large increase last year – what’s next step for this year
        1. Letter was sent from the clinics without response
        2. Increase should be minor if any for most this year
  • Changes would need to occur at the OHA board level
  1. Examine overall value for dollar re: access to HOOPP and retention – creative budgeting, etc
  1. Beth – Primary Care Council has met once – preliminary – confidentiality agreement is in place; public information is being shared re: upcoming changes to the LHINs although details are not yet know – may have further updates at the next meeting and any public information will be shared
