2.0 Introduction of Guests – BC NPLC Project

Leanne Rowand is a British Columbia NP who is involved in establishing 3 NPLCs in BC to be located within primary care networks in Surrey, Victoria and Naniamo. She spoke on behalf of 2 other NPs, Lynn Guengerich and Lisa Dawson.  There was an open discussion between Leanne and the Ontario NPLC leaders on the call with regard to advice in start up.  Key points shared included:

  • Ask for funding for everything you think you will need for your clinic at start up.  Otherwise it is difficult to obtain additional funding later on
  • Ensure data is standardized from the beginning
  • Engage area physicians, even if they are resistant
  • Don’t re invent the wheel

3.0 Review of previous minutes

Not all NPLC leaders are able to access the minutes on the NPLCA micro-site

Sue Tobin explained that minutes are not sent out but are instead posted.  Minutes of NPLCA meetings are only available to NPLCA members, but minutes of joint NPLCA/NPLC/QIIMS meetings will be available to everyone.

Action: 

Sue Tobin will look into facilitating access to the minutes on the NPLCA micro-site for those not able to access them

4.0 QIIMs Update on Workplan

Presenters:  Kathryn Nicholson, Elyse Lamontagne, Al Syed          

The QIIMs provided an update on their workplans.  Their current projects include

  • Standardizing and Cleaning Diagnostic Codes within the EMR (problem list)
  • Quarterly Reporting
  • Patient Experience Survey
  • Social Determinants of Health Indicators Data Collection
  • Completing profiles of each NPLC (The NPLC Story)

The QIIMS answered questions from the NPLCs in relation to this work.

In response to the urgent need for NPLC data relating to Ontario Health Team applications and submissions, the QIIMS will prioritize Standardizing and Cleaning Diagnostic Codes within the EMR (problem list) for the fall 2019.   Suggestions on how to complete this work were shared (use interprofessional team, employ a student).  Although the QIIMS can support the NPLCs to complete this work, it is beyond their capacity to input the codes on behalf of the clinic.  Therefore responsibility to enter the standardized codes is with each NPLC.

Action: 

All NPLCs will develop standardized problem lists with IDC-10 (preferred), IDC-9 or ENCODEs with a goal of November 30 2019 for completion.  

5.0 Ontario Health Teams and Patient Enrollment

There are 10 NPLCS who are involved in OHTs that were asked to submit a full –application, due Oct 5 2019.   North Muskoka, Georgian, North Bay, Waterloo-Wellington and Ingersoll shared their experience with and challenges the OHT process.  Although some NPLCs had difficulty being included in OHT planning initially, these challenges have resolved.  

There is concern about the Ministry of Health’s plan to fund OHTs through geographic attribution, and to add NPLC patient numbers to an area physician as a result of our inability to attach our patients to their NP or NPLC.    NPAO and the NPLC have addressed this issue with the Ministry of Health representatives, with the Chief Nursing Officer (Michelle Acorn) and ICES.   There is considerable movement toward Enrollment of patients with their NP.   The term rostering, which is a financial arrangements between the ministry and  Family Health Team physicians, was compared to the term  Enrollment, which is formal connection between the NP and their patients that facilitates collecting “big picture “ data on our patients, allows for access to Cancer Care Ontario data and My Practice reports. 

ACTION: 

NPLCS involved in OHTs will continue to communicate with the NPLCA executive and other NPLCS to identify issues relating to NPLC practice and operations. 

NPLC leaders asked to use the term Enrollment instead of Rostering when addressing patient attachment to NPLCS and their NPs.
